In normal anatomy, the anterior tibial artery is typically first branch of popliteal before it becomes tibioperoneal trunk. The course includes piercing through interosseus membrane and continuing compartment. It then continues onto dorsum foot as dorsalis pedis at level malleoli. We describe a unique case an anomalous origin from peroneal artery. important for vascular surgeons to be aware this variant while interpreting arteriograms lower extremity. can easily misinterpreted occluded distal with reconstitution collaterals.
